Initial Consultation | Continuum of Care | Medication Adjustments | Detox/Outpatient Opioid Reduction
Initial Follow-up -Detox/Opioid Reduction | Continuum of Care -Detox/Opioid Reduction

Initial Consultation | Chronic Pain - $225

  • Comprehensive examination by a staff physician
  • Evaluation of medical history and records
  • Drug Screening
  • Set personal goals
  • Development of individualized pain management plan
  • Assessment guidelines for continuum of care
  • Thorough explanation of appropriate medications
  • Office visit: 45 to 60 minutes as needed

If necessary:

  • Additional screening and diagnostic testing referrals (MRI, etc.)
  • Referrals to nonpharmacological treatment facilities (physical therapy, etc.)

Continuum of Care |Chronic Pain - $195

  • Meet with physician and other pain management professionals
  • Evaluation of individualized pain management plan
  • Review of personal goals and quality of life enhancements, including work activities/requirements and social interactions/relationships
  • Evaluation of any new testing or reports
  • Continuation of pharmacological protocol
  • Office visit every 28 days required

If necessary:

  • Referrals for additional testing or evaluation
  • Adjustment of medication protocol

Medication Adjustments - TBD

  • Meet with physician between regularly scheduled appointments
  • Reevaluation of medication and its effectiveness
  • Adjustment of pain management plan
  • Cost to be determined on an individual basis

IMPORTANT!

  • ALL remaining medications that were prescribed at your pain center MUST be brought in with you for any Medication Adjustment appointment. NO EXCEPTIONS.

Initial Consultation |Outpatient Detox Opioid Reduction (Day One) - $139

  • Comprehensive evaluation by an SAMHSA certified physician
  • Drug Screening
  • Assessment of personal goals
  • Discreet and confidential process
  • Continual supervised care and assessment during entire day long process
  • Individualized titration plan to taper off medications
  • Thorough explanation of appropriate medications
  • Referrals to additional healthcare support facilities

Initial Consultation Follow-up | Outpatient Detox Opioid Reduction (Day Two) - $139

  • Follow-up visit day after the initial consultation
  • Meet with physician for progress review
  • Evaluation of individualized titration plan
  • Review of personal goals and quality of life enhancements
  • Continuation of pharmacological protocol

Continuum of Care | Outpatient Detox Opioid Reduction - $55

  • Meet with physician for progress review
  • Evaluation of individualized titration plan
  • Review of personal goals and quality of life enhancements
  • Continuation of pharmacological protocol
  • Weekly office visit required
